**Job Description:**

- Serve as a treatment team member, conducting advanced practice individual and group clinical interventions as prescribed on individual mental health treatment plans.
- Utilize clinical intervention strategies within the scope of practice directed by the Behavioral Healthcare Supervisor or Mental Health Administrator.
- Provide clinical case management services to an assigned caseload, including monitoring compliance with mental health treatment plans.
- Complete biopsychosocial assessments and mental health status examinations as part of interdisciplinary treatment planning.
- Prepare transfer and discharge summaries and SSI requests.
- Consult with other institution staff regarding institutional adjustment and service needs.
- Initiate and maintain contact with families and community agencies as necessary for patient care pre-incarceration.
- Make appropriate community and re-entry referrals as per mental health treatment team.
- Document all encounters in the electronic healthcare record and assist with discharge, release, and wellness plan development.
- Complete miscellaneous forms and reports per policy and protocol.
- Attend meetings with supervision and participate in in-service training sessions and workshops for professional development (CEUs).
- Participate in peer reviews as assigned.
- Update patient mental health classifications and perform supportive functions.
