MUST- HAVES
Must be a certified Licensed Nurse Practitioner. An RN with at least a Master Degree in Nursing and advanced education in primary care.

Must possess a dependable automobile with insurance and a valid California drivers license. Must submit to and pass a urinalysis test, fitness for duty, criminal background and credentialing clearance as a precondition for employment. Must be able to provide proof of COVID-19 vaccination and booster, recent TB test, MMR, TDAP and Hepatitis B vaccine.

JOB DETAILS
  • The Family Nurse Practitioner provides generalized health care services; is under the supervision and reports directly to the supervising licensed physician.
  • The Nurse Practitioner assumes principle responsibility, functioning with approved protocols for health maintenance and clinical management of acute and chronically ill patients.
  • Identifies the health status of patients by taking medical histories, performing examinations and initiating and evaluating screening and diagnostic procedures.
  • Initiates appropriate therapeutic intervention based upon assessment and evaluation of the health status of the individual.
  • Determines which cases to refer to the licensed physician for further diagnosis and treatment; serves as clinical consultant to nursing and paramedical personnel; and coordinates work activities with families, community agencies and other members of the health.
  • The Family Nurse Practitioner is a member of the patients care team and is involved in the coordination of patient care.
  • Participates in annual trainings, such as: CME and the Patient Centered Medical Home (PCMH).
  • Performs other duties as assigned.
