Job Title: Occupational Therapist (OT)

Location: Sonoma, California area

Overview: Soliant Health is currently seeking a skilled and passionate Occupational Therapist to join our team in the beautiful Sonoma, California area. As an Occupational Therapist with Soliant Health, you will play a vital role in helping individuals regain independence and improve their quality of life. Qualifications:

  • Master's degree (or equivalent) in Occupational Therapy from an accredited program
  • Current valid license as an Occupational Therapist in the state of California or eligibility to obtain licensure
  • Previous experience in a school setting is preferred
  • IEP experience is preferred but not required

Responsibilities:

  • Conduct prehensive evaluations to assess patients' functional abilities and limitations
  • Develop individualized treatment plans based on patients' goals, needs, and abilities
  • Implement evidence-based therapeutic interventions to improve patients' independence in activities of daily living, mobility, and fine motor skills
  • Provide education and training to patients, families, and caregivers on therapeutic techniques and adaptive strategies
  • Collaborate with other healthcare professionals, including physicians, nurses, and physical therapists, to ensure coordinated care and optimal patient outes
  • Maintain accurate and timely documentation of patient evaluations, treatment plans, progress notes, and discharge summaries
