Job Overview:
LHH Recruitment Solutions is excited to partner with a well-respected organization in Indianapolis to find a compassionate and skilled Case Manager. This full-time role offers you the opportunity to make a significant impact by providing vital care coordination and support to individuals in need. As a Case Manager, you will work with clients to help them navigate complex healthcare systems and connect them with essential services, ensuring they receive the best possible care.

Key Responsibilities:

  • Assess client needs and develop personalized care plans

  • Coordinate medical, behavioral, and social services for clients

  • Act as a liaison between clients, healthcare providers, and community resources

  • Advocate for clients to ensure access to necessary services and support

  • Monitor client progress and adjust care plans as needed

  • Maintain detailed and up-to-date case documentation

  • Educate clients and their families on available services and resources

  • Collaborate with interdisciplinary teams to deliver comprehensive care

Qualifications:

  • Bachelor’s degree in Social Work, Nursing, or a related field (Master’s preferred)

  • Prior experience in case management, care coordination, or a related field

  • Knowledge of local healthcare systems and resources

  • Strong communication and organizational skills

  • A client-centered, empathetic approach to care

  • Ability to work independently and within a team

Pay Details: $20.00 to $26.00 per hour
