Job Overview:

As a Clinical Pharmacist, you will play a vital role in patient care by:

  • Documenting drug information questions, drug utilization evaluation activities, and adverse drug reactions.
  • Understanding and addressing the unique needs of neonatal, pediatric, adolescent, adult, and geriatric patients through effective documentation in progress notes and pharmacy care plans.
  • Ensuring adherence to departmental policies, procedures, and State and Federal laws by pharmacy interns, residents, and technicians.
  • Promoting adherence to the Drug Formulary approved by the Pharmacy and Therapeutics mittee.
  • Ensuring accurate charging documentation for services provided.
  • Promoting and marketing the pharmacy department to the munity, patients, hospital employees, and medical staff.
  • Maintaining positive, open munication with members of the Medical Staff and their teams.
  • Upholding clinical petency through self-study modules and hands-on practice.
  • Teaching, directing, and supervising pharmacy students, residents, and medical assistants.
  • Actively participating in the development and implementation of new programs while supporting existing initiatives.
  • pleting assigned petency assessments and additional responsibilities/projects as directed by the Pharmacy Director.

Preferred Education:

  • pletion of an advanced degree is preferred but not required.
  • pletion of an ASHP Accredited Residency and/or Fellowship is also preferred but not mandatory.

Required Experience:

  • Demonstrated experience in anticoagulation therapy, bridge therapy, and drug information.
