Clinical Pharmacist

This role is a dedicated transitions of care pharmacist focused on admission medication reconciliation for hospitalized patients. The position oversees technicians and precepts students and residents. A strong interest in transitions of care, teaching, quality improvement, and leadership development is preferred.
Key Responsibilities
Conduct thorough medication reconciliation for patients upon admission
Collaborate with healthcare teams to ensure accurate medication management
Provide education and training to students and residents
Develop and implement quality improvement initiatives
Requirements
PharmD Doctorate of Pharmacy or Bachelor's Degree in Pharmacy
Washington Pharmacist License
Basic Life Support-Provider (BLS) certification
Preferred Qualifications
Acute care hospital pharmacy experience
