Job Title: Licensed Vocational Nurse

Job Summary:

The Licensed Vocational Nurse will provide primary care services to patients who meet criteria for services under the Certified Community Behavioral Health Center (CCBHC). The LVN will work closely with internal La Familia teams, key stakeholders, community organizations, and county social workers to consult and collaborate on client progress and treatment plans.

Key Responsibilities:
  • Conduct assessments, planning, implementation, and evaluation of patient care
  • Make determinations whether a patient needs a physician or should be sent to the emergency room
  • Recognize, report, and record any change in condition or behavior of the patient
  • Accurately communicate in writing through the Electronic Health Record, the patient care summary, and proper transcription of medical orders
  • Establish therapeutic rapport with patients, families, and walk-in community members, while respecting patient's rights to privacy and confidentiality of information
  • Communicate pertinent information regarding patients with the team
  • Support patients in registering on the testing lab website, refer out for COVID-19 testing, and place follow-up phone calls to communicate test results and/or discrepancies
  • Provide patient information as necessary, complete and document medication count, and monitor patient's post-administration for adverse effects
  • Prepare and perform blood pressure screenings, A1C and blood glucose screenings, weight assessment, and counseling for nutrition and physical activity
  • Take, monitor, and control High Blood Pressure for clients, provide patient education and supporting materials, and train and educate adults and families on psychiatric education
  • Participate in program planning and utilization review, quality assurance, and follow Drug Medi-Cal Organized Delivery System charting and documentation requirements and Agency regulations
Requirements:
  • Active license with the California Board of Registered Nursing
  • Bachelor of Science in Nursing or completion of a license-eligible program in nursing
  • Registered Nurse or active license with the California Board of Registered Nursing
  • Current Basic Life Support Certification
  • Strong customer service skills, organizational and communication skills, and ability to organize, prioritize, multi-task, and adapt to changing priorities
  • Knowledge and application of clean technique and sterile technique
Preferred Qualifications:
  • Spanish, Mam, Mandarin, Farsi, Arabic bilingual
  • Full-time availability
  • Advanced Care Life Support Certification
  • Experience driving a company vehicle
  • Valid CA Driver's license
