- Oversee and coordinate comprehensive care for patients and their caregivers.
- Conduct thorough nursing assessments to address patient and caregiver needs.
- Develop, implement, and revise care plans to ensure continuity and achievement of care goals.

- Utilize established protocols and guidelines for hospice care effectively.
- Provide education to patients and families, facilitating understanding and engagement.

- Participate in and contribute to interdisciplinary team meetings and care discussions.

- Ensure clinical documentation is accurate, complete, and compliant with organizational standards.

- Demonstrate ongoing competency in nursing skills and knowledge.
- Maintain professional licensure and certifications, including CPR.
- Stay informed about current trends in healthcare and integrate new practices into care.
- Foster positive relationships with external healthcare providers.
- Additional Duties
- Maintain cleanliness and organization of patient care areas, ensuring all supplies are stocked and ready for use.
Required Qualifications
- Associate Degree in Nursing; Bachelor's Degree preferred.
- Active and unrestricted Registered Nurse (RN) license.
- Current CPR certification; CHPN certification preferred.
- Minimum of 1 year of experience in inpatient settings and 2 years in other nursing roles, particularly with patients facing life-limiting illnesses.
- Proficiency in computer skills is essential.
