Key Responsibilities:- Provides Patient Care: Develop and implement individualized care plans to meet the unique needs of each patient, ensuring safe and effective infusion therapy.
- Collaborates with Healthcare Team: Work closely with primary care physicians, clinical teams, and other healthcare professionals to ensure seamless care coordination and communication.
- Conducts Home Visits: Perform regular home visits to assess patient needs, provide education, and ensure proper infusion therapy administration.
- Documents Patient Information: Accurately and thoroughly document patient information, including medical history, medication, and treatment plans.
- Communicates Effectively: Develop and maintain strong relationships with patients, families, and healthcare teams through effective communication and empathy.
- License and Certification: Current registration to practice as a Registered Professional Nurse in New York State and CPR/BLS certification.
- Education: Associate's Degree in Nursing from an approved program required; Bachelor's Degree in Nursing from an approved program preferred.
- Experience: Minimum of two years in a clinical background as a Registered Nurse and one-year clinical practice in specialty where appropriate.
