Job Description

VNS Health is seeking a skilled Nurse Care Manager to join our team in providing high-quality healthcare services to our clients.

Key Responsibilities:
  • Collaborate with interdisciplinary care teams to develop and implement comprehensive care plans for residents.
  • Conduct thorough assessments of residents' health needs and provide interventions to promote successful community living.
  • Provide health-focused outreach to seniors who may benefit from supportive services.
  • Implement health promotion activities, such as health screenings, health assessments, and health care referrals.
  • Make telephone calls and home visits to assess residents' needs and support successful community living activities.
  • Refer residents to appropriate health services based on their assessed needs.
  • Provide healthcare advocacy by acting as a liaison between healthcare systems during times of transition.
  • Participate in quality assurance and education programs as requested by the Clinical Manager.
  • Document all nursing interventions provided as required.
Requirements:
  • License and current registration to practice as a Registered Professional Nurse in New York State.
  • CPR/BLS Certification.
  • Associate's Degree in nursing required; Bachelor's Degree in nursing preferred.
  • Minimum of two years of experience working as an RN in a Long-Term Home Care Services Agency (LHCSA) or Continuing Care Health Facility (CHHA).
  • Prior experience working with an aging population and providing clinical assessments of their health needs.
