Key Responsibilities- Provide individualized care to pediatric patients in their homes, focusing on their unique needs and medical requirements.
- Develop and implement personalized care plans, working closely with patients, families, and healthcare teams to ensure seamless care coordination.
- Administer medications, perform medical procedures, and provide wound care, as needed.
- Monitor patients' conditions, reporting any changes or concerns to the healthcare team.
- Collaborate with other healthcare professionals to ensure comprehensive care and support for patients and families.
- Current valid license to practice as a Licensed Vocational Nurse (LVN) or Registered Nurse (RN) in the state of Texas.
- Current CPR certification.
