About the Role

We are seeking a skilled Occupational Therapist to join our team at CenterWell Home Health. As a key member of our care team, you will play a vital role in helping patients manage chronic conditions and recover from illness or injury in the comfort of their own homes.

Key Responsibilities
  • Assess and develop personalized rehabilitation plans for patients to improve their daily living and work-related skills.
  • Administer and interpret diagnostic tests to evaluate patient function and progress.
  • Collaborate with healthcare teams to develop and implement treatment plans that meet patient needs.
  • Provide therapeutic treatment and instruction to patients to improve strength, coordination, and range of motion.
  • Review and expand treatment plans to ensure patient progress and goals are met.
  • Consult with other healthcare professionals to develop vocationally oriented plans for patients.
  • Report patient status and progress to healthcare teams and family members.
Requirements
  • Master's degree from an accredited Occupational Therapy program.
  • Minimum of six months of occupational therapy experience, with home health experience a plus.
  • Current and unrestricted OT licensure.
  • Current CPR certification.
  • Good organizational and communication skills.
  • A valid driver's license, auto insurance, and reliable transportation.
