About the Role

We are seeking a highly motivated and compassionate Community Integration Worker to join our team at Ascentria Care Alliance. As a Community Integration Worker, you will play a vital role in supporting individuals in achieving their personal and social goals, while promoting their independence and well-being.

Key Responsibilities

  • Provide direct support and advocacy to individuals served, ensuring their needs are met and their rights are respected.
  • Assess and develop individualized plans of care, taking into account their unique needs and goals.
  • Establish and maintain relationships with community partners, including public and private agencies, to ensure access to necessary services and supports.
  • Conduct regular assessments and evaluations to ensure the effectiveness of individualized plans and make adjustments as needed.
  • Ensure accurate and timely documentation of all services and interactions with individuals served.

Requirements

  • High School Diploma or equivalent required; related associate's degree preferred.
  • 6 months of related experience desired.
  • Valid State-issued Driver's License and proof of current insurance required.
  • MHRT-C or MHRT-C Provisional certification required; must maintain with 18 approved CEU's every 2 years.
