Senior Nurse Practitioner

We are seeking a highly skilled Senior Nurse Practitioner to join our Senior Community Care team at UnitedHealth Group. As a key member of our interdisciplinary team, you will play a vital role in providing high-quality, patient-centered care to our most vulnerable populations.
Key Responsibilities- Conduct comprehensive assessments and develop personalized care plans to meet the unique needs of our patients.
- Manage complex medical situations, including acute and chronic conditions, and provide timely interventions to ensure optimal patient outcomes.
- Collaborate with our interdisciplinary team to ensure seamless care coordination and communication.
- Provide advanced illness and end-of-life discussions, as well as counseling and education to patients and their caregivers.
- Ensure accurate and complete documentation, including ICD-10 conditions, to support high-quality care and reimbursement.
- Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program.
- Active and unrestricted license in the state where you reside, or ability to obtain by start date.
- Active Nurse Practitioner certification through a national board, such as the American Academy of Nurse Practitioners or the American Nurses Credentialing Center.
- Current, active DEA licensure/prescriptive authority or ability to obtain post-hire, per state regulations.
- Ability to gain a collaborative practice agreement, if applicable in your state.
- Ability to complete physical requirements of the job, including lifting a 30-pound bag in and out of the car, navigating stairs, and sitting, standing, and kneeling to perform physical assessments.
- Access to reliable transportation.
- 1+ years of clinical experience in a long-term care setting.
- Experience in meeting the medical needs of patients with complex behavioral, social, and/or functional needs.
- Understanding of Geriatrics and Chronic Illness.
- Understanding of Advanced Illness and end-of-life discussions.
- Proficient computer skills, including the ability to document medical information with written and electronic medical records.
