Job Summary

Horizon Home Care & Hospice is seeking a dedicated and compassionate Home Health Aide to join our team. As a Home Health Aide, you will provide essential support to patients in their homes, promoting their health and well-being under the guidance of a Registered Nurse or Therapist.

Key Responsibilities
  • Assist patients with daily living activities, such as bathing, dressing, and toileting, under the supervision of a Case Manager.
  • Review care plans with patients and involve them in their care, ensuring their comfort and understanding.
  • Complete mobile care tasks, including mileage logs, in accordance with Agency policy.
  • Work collaboratively with patients, care providers, and family members to meet their needs and provide excellent care.
  • Perform regular home maintenance tasks, such as house cleaning, laundry, and meal preparation, as outlined in the care plan.
