Job Overview

CenterWell Home Health is seeking a skilled Speech Language Pathologist to join our team. As a Home Health Speech Language Pathologist, you will play a vital role in providing high-quality care to patients in their homes.

Key Responsibilities
  • Evaluate and develop individualized plans of care for patients with speech, language, and swallowing disorders.
  • Conduct thorough assessments and provide evidence-based interventions to improve patient outcomes.
  • Collaborate with interdisciplinary teams to ensure comprehensive care and coordinate services.
  • Provide education and training to patients, families, and caregivers on communication strategies and techniques.
  • Document patient progress and maintain accurate records.
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures.
Requirements
  • Master's degree in Speech-Language Pathology or related field.
  • Current certification from the American Speech-Language-Hearing Association (ASHA).
  • Minimum of six months of experience as a speech-language pathologist in a home health setting.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
