Job DescriptionAt Hill Country MHDD Centers, we are seeking a skilled Care Coordinator to join our team. As a key member of our mental health services team, you will play a vital role in assessing the needs of individuals and developing personalized treatment plans. Your expertise will help individuals and their families navigate the mental health system, providing them with the support and resources they need to achieve their goals.

Key Responsibilities:
  • Assess individual needs and develop effective treatment plans in collaboration with the individual and their family members.
  • Provide skills training and education to improve daily life and promote independence.
  • Collaborate with service providers and agencies to ensure seamless care and positive outcomes.
  • Document progress and maintain accurate records.
  • Ensure compliance with all legal requirements and company policies and procedures.

Requirements:
  • At least one year of experience in mental health services or a related field.
  • A bachelor's degree in psychology, sociology, or a related field.
  • Valid Texas State Driver's License and current automobile insurance.
