Position Overview

The Clinical Nurse Navigator/Coordinator serves as an integral member of a multidisciplinary team, bringing clinical nursing expertise to a designated area. This role emphasizes both clinical and educational dimensions of patient care, acting as a clinical resource, advocate, and educator. The Clinical Nurse Navigator is actively present in the clinic, contributing significantly to the care team.

Key Responsibilities

To excel in this position, the individual must effectively carry out the following essential duties:
  • Exhibit a strong dedication to patient-centered care within an interdisciplinary framework.
  • Coordinate follow-ups with patients regarding testing and procedure outcomes in collaboration with healthcare providers.
  • Prepare clinic preparation notes for both new and returning patients, which includes scanning notes/imaging, liaising with referring physicians or imaging groups, and addressing any patient concerns prior to visits.
  • Assist in organizing follow-up appointments, orders, and referrals.
  • Engage in quality improvement initiatives, patient safety measures, and enhancing patient experience.
  • Contribute to the training and education of support staff.
Qualifications

Education
  • An Associate's degree or higher in Nursing, or a comparable combination of training and experience.
Licensure
  • A current and valid D.C. license as a Registered Nurse or proof of application for licensure in D.C.
Experience
  • A minimum of 2 years of experience as a Registered Nurse.
Physical Requirements
  • Ability to sit, walk, reach, bend, or twist for extended periods in a clinical environment.
  • Must be capable of lifting, carrying, pushing, or pulling up to 100 lbs. as part of the responsibilities.
  • Regular exposure to healthcare settings that may necessitate personal protective equipment.
  • Requires manual dexterity for operating a computer keyboard, calculator, copier, and other equipment.
