Job Summary

We are seeking a skilled Licensed Practical Nurse to join our Family Medicine Clinic team at UnityPoint Health. As a key member of our clinical staff, you will provide high-quality patient care and support our providers in delivering exceptional patient experiences.

Responsibilities:

  • Assist providers with obtaining vital information and preparing patients for examination, treatment, and minor surgical procedures.
  • Track follow-up appointments and monitor disease status.
  • Schedule referrals for patients with specialists or outpatient services.
  • Complete forms and requisitions for referrals and lab testing.
  • Provide teaching and support to patients and family members.
  • Facilitate relationships between providers and ancillary services.

Requirements:

  • Graduate of an accredited program for Licensed Practical Nurses.
  • Current license to practice nursing in the State of Illinois.
  • Previous clinical experience in a medical office preferred.
  • BLS and Mandatory Reporter certification.
  • Computer data entry experience.
  • Strong interpersonal skills.
