We are seeking a Certified Peer Specialist/Peer Care Manager to join our team at Human Development Services of Westchester, Inc. This role will work in concert with the Director of CORE & HCBS and the Team to provide support, services, and link Members to resources in their home or community.
Key Responsibilities- Provide person-centered, trauma-informed care to Members, emphasizing and supporting their potential for recovery.
- Optimize quality of life and reduce symptoms of behavioral health conditions through empowerment, choice, and health and wellness goals.
- Collaborate with the Director of CORE & HCBS and the Team to develop and implement individualized care plans.
- Link Members to community resources and services to support their recovery and well-being.
The ideal candidate will have a strong understanding of person-centered, trauma-informed care and experience working with vulnerable populations. A Certified Peer Specialist certification is required, and a bachelor's degree in a related field is preferred.
We offer a competitive hourly wage of $22-24 and a dynamic work environment that values employee growth and development.
