We are seeking a highly skilled Diabetes Educator II to join our team at Christus Health. As a key member of our healthcare team, you will be responsible for providing high-quality diabetes education and support to patients and their families.
Key Responsibilities:
- Develop and implement effective diabetes education programs for patients and their families.
- Conduct individual and group education sessions on diabetes management, nutrition, and lifestyle changes.
- Collaborate with healthcare providers to ensure seamless care coordination and patient outcomes.
- Provide ongoing support and education to patients and their families through phone calls, emails, and in-person visits.
- Stay up-to-date with the latest diabetes research, guidelines, and best practices to ensure evidence-based care.
- Participate in multidisciplinary teams to develop and implement diabetes care plans.
- Document patient progress and outcomes to inform care decisions and quality improvement initiatives.
Requirements:
- Bachelor's degree in a related field, such as nursing, health education, or a related field.
- 0-2 years of experience in diabetes education or a related field.
- BLS (Basic Life Support) certification required.
- RD (Registered Dietitian) or CDCES (Certified Diabetes Care and Education Specialist) certification preferred.
