Job Summary:

Nurse Care Coordinator is a key role at Philadelphia FIGHT, a Federally Qualified Health Center (FQHC) providing comprehensive care to diverse patients. We seek a skilled nurse to join our team, working closely with primary care staff, patients, and behavioral health counselors. The ideal candidate will have a strong background in nursing, excellent interpersonal skills, and the ability to work collaboratively in a fast-paced environment.

Responsibilities:

Triage sick/emergency walk-ins and calls
Communicate normal and abnormal labs to patients and recommend follow-up based on medical provider recommendations
Assist with one-on-one management and teaching of complicated patients with multiple diagnoses, complicated medical regimens, and/or complex social needs
Identify and advocate for patients needing urgent or ongoing behavioral health services and/or substance abuse treatment via collaboration with the primary provider and/or onsite Behavioral Health Consultant
Administer therapeutic injections and vaccinations and perform phlebotomy when needed
Perform clinical tasks, including EKG, oxygen setup, blood glucose monitoring, urine dipsticks, and pregnancy testing as backup to clinical medical assistant
Coordinate ordering of clinical supplies, including monitoring for expiration of supplies and maintenance of clinic medical equipment
Work as a team with medical receptionists, health information specialists, medical assistants, benefits coordinators, and behavioral health consultants

Requirements:

A degree in nursing is required, and a Registered Nurse license for the state of Pennsylvania is necessary. The ideal candidate will have a general knowledge of science, medicine, and the running of a medical clinic. Excellent interpersonal skills and the ability to work collaboratively with primary care staff, patients, and other behavioral health counselors are essential. The nurse must be able to demonstrate judgment, patience, integrity, organization, and prioritization in a multi-task environment. Attention to detail, dependability, and flexibility are also required.

Philadelphia FIGHT offers a highly competitive benefits package, including 15 vacation days per year, 12 sick days per year, 11 paid holidays, 403B with employer match, medical, vision, and dental insurance starting day 1, and FSA with an annual employer contribution for medical. HRSA-approved site with loan repayment options available. Initial probationary period is three months. Performance of other tasks may be assigned on an as-needed basis.
