Position Overview

Under the supervision of senior staff and within established systems, protocols, and schedules, the MDS Coordinator will receive and input information from the Minimum Data Set (MDS) to ensure accurate and timely data collection and reporting.

Key Responsibilities
  • Receive and input MDS information from various sources, including resident assessments and care plans.
  • Ensure accurate and timely data collection and reporting, adhering to regulatory requirements and facility policies.
  • Collaborate with interdisciplinary teams to ensure comprehensive care planning and delivery.
  • Develop and maintain relationships with healthcare professionals, residents, and families to promote open communication and collaboration.
  • Participate in quality improvement initiatives to enhance resident care and services.
Requirements
  • Current and unencumbered CA RN or LVN license.
  • Minimum 6-month training experience in rehabilitative and restorative nursing practices.
  • MDS Certified preferred.
  • Proficient in PDPM.
  • High School education or GED.
