STGi is seeking a highly skilled Registered Nurse Care Manager to join our team at our Community Based Outpatient Clinic. As a key member of our healthcare team, you will be responsible for providing exceptional patient care and coordinating services with VA medical centers and stakeholders.
Key Responsibilities:- Provide professional clinical services under the direction of the Community Based Outpatient Clinic (CBOC) and manage patient care.
- Supervise support staff and lead PACT team huddles to ensure seamless care delivery.
- Work with Primary Care Physicians to ensure adequate staffing and continuity of care between the VAMC and the clinic.
- Maintain universal precautions and infection control practices.
- Administer medications, perform cardiopulmonary resuscitation, and assist during emergency procedures.
- Document patient information in medical records and VISTA to demonstrate quality care delivery.
- Participate in Performance Improvement Programs and ensure compliance with federal, state, and local regulations.
- Minimum of 3 years of experience in Medical Surgical Unit, Outpatient Facility, Primary Care, and/or Adult Health.
- Current and unrestricted nursing license in the state of practice.
- Current Cardiopulmonary Resuscitation (CPR)/Basic Life Support (BLS) certification.
- Knowledge of professional nursing principles, procedures, and techniques.
- Ability to assess patient condition, recognize adverse signs, and react swiftly in emergency situations.
- Competitive hourly rate of $31.61.
