Key Responsibilities:
  • Assist providers with patient care administration, ensuring seamless delivery of services.
  • Serve as a liaison between providers and patients, assessing needs and communicating health information.
  • Perform telephone triage, accurately assessing symptoms and communicating with patients and providers.
  • Follow service line initiatives to prevent unnecessary hospital re-admissions, ensuring timely follow-up and communication.
  • Assist with medication management, performing medication reconciliation and queueing necessary refills.
  • Maintain a safe and healthy work environment, adhering to internal and legal regulations.
  • Complete prior authorizations for medication and diagnostic services, ensuring timely processing.
  • Monitor incoming documents and assist with the completion of necessary forms.
  • Deliver exemplary customer service, providing a positive patient experience across the organization.
Requirements:
  • Applicable NY State license, two-year degree preferred.
  • Proven work experience in a healthcare facility, preferred.
  • Basic knowledge of ICD-10 and CPT Codes, CPR Certification, and strong clinical skills, preferred.
  • Strong written and verbal communication skills, with the ability to utilize computer programs.
