Job Title: Assessment Clinician

At Turning Point Community Programs, we are seeking a highly skilled and compassionate Assessment Clinician to join our team. As a key member of our clinical staff, you will play a vital role in providing psychosocial, rehabilitation, and recovery-oriented services to adults.

Job Summary

The Assessment Clinician will be responsible for ensuring that clients are introduced into the program in a comfortable, thorough, and timely manner. This includes completing the initial assessment of clients to confirm eligibility under the target population and keeping clients and families engaged in services until assignment to a primary therapist is successful.

Key Responsibilities
  • Complete the intake Core Assessment, including collateral assessments (CANS, CHQ, MSE) in the field or in the office.
  • Support the development of the Individual Client Plan in collaboration with the member and the treatment team through linkage and handoff at CFT.
  • Identify strengths and challenges of the child and family system.
  • Screen for barriers to treatment.
  • Identify the child and family's natural supports.
  • Interact productively with the treatment team to provide holistic assessment and engagement services that are member-driven and recovery-oriented.
  • Complete all required paperwork as assigned in a timely manner.
  • Provide prompt intervention in the event of a crisis, both to stop the crisis and to notify, when indicated, persons and agencies necessary for the resolution of the crisis situation.
  • Provide on-the-spot counseling that is both helpful to the clients and consistent with the philosophy of the program.
  • Initiate contact with previous providers, Primary Care Physicians, Psychiatric Hospitals, and/or collateral resources to obtain assessment information and provide case management during transition as necessary.
  • Ensure that clients discharging from psychiatric hospitals are linked to the agency's Psychiatrist within 30 days. Ensure that clients transferring to services with medication already in place are linked to the agency's Psychiatrist.
  • Develop initial safety plans and monitor safety planning in high-risk cases during the assessment period.
  • Complete Service Requests and link clients and families to community resources as appropriate.
  • Facilitate groups and provide individual and family therapy.
  • Support the Family Advocate with engagement of new referrals to the program.
Qualifications

MINIMUM:

A typical way of obtaining the knowledge, skills, and abilities outlined above is through graduation from an accredited college or university with a master's degree in Social Work or a related field; a minimum of 4 years' experience as a provider of mental health services, and valid registration with the Board of Behavioral Sciences (BBS).

LICENSES; CERTIFICATES; SPECIAL REQUIREMENTS

  • California driver's license & current vehicle insurance/registration
  • Reliable means of transportation capable of passing vehicle safety inspection if more than five years old excluding all modes of two-wheeled transport inclusive of bicycles, mopeds, and motorcycles.
  • Valid registration with the Board of Behavioral Sciences (BBS)

Schedule: Thursday-Monday, 4:00 PM - 12:30 AM
