Job Summary:

Cary Medical Center seeks a skilled Clinical Social Worker to provide professional clinical social work services to patients admitted to our hospital and in outpatient areas. The ideal candidate will evaluate case situations, present information to the multidisciplinary team, and facilitate adjustment of patients to the impact of illness.

Essential Job Duties:
  • Provide professional social work services to patients admitted to the hospital and manage referrals from the outpatient setting.
  • Facilitate adjustment of patients to the impact of illness and promote maximum benefit from healthcare services.
  • Evaluate case situations and present information to the Interdisciplinary Team.
  • Ensure that care is delivered with respect for the individual's rights and preferences.
  • Maintain high standards of patient care using professional specialty standards of care and practice.
  • Prioritize a safe environment for all patients.
  • Evaluate patients with psychiatric and/or mental disorders and identify strategies for management while in the hospital.
  • Makes appropriate referrals and follow-up care.
  • Provide case management functions to new mothers under age 18 and their infants.
  • Understand the needs of the elderly and provide discharge planning to nursing homes, boarding homes, and rehabilitation services.
  • Coordinate patient care activities with other departments contributing to patient care.
Requirements:
  • Must have an active, valid Maine Social Work license.
  • Must obtain BLS certification within 1 year of hire and maintain current certification thereafter.
