LSI is currently seeking a Case Management Program Coordinator to oversee services for clients requiring intensive and extended case management. This role is essential in ensuring that our team delivers comprehensive, client-focused, and culturally appropriate care.

Key Responsibilities

  • The Case Management Program Coordinator will identify and coordinate services for clients with complex case management needs.
  • Ensure that quality assurance policies and procedures are established to comply with funders' and contractual obligations.
  • Act as an advocate for clients, assisting in service coordination to support their independent living plans.
  • Provide additional resources to staff, particularly regarding crisis intervention.
  • Develop and nurture community partnerships to guarantee that clients have access to necessary social services.
  • Plan and implement programming, supervise staff, and ensure that program objectives are achieved while adhering to budgetary, compliance, and reporting standards.
  • Oversee service provision during regular business hours, after hours, or on weekends as needed.
  • Manage relationships with both internal and external partners to facilitate effective service delivery.

Qualifications

  • A Bachelor's degree in a health or human services-related field, such as psychology, sociology, counseling, or human/family services.
  • A minimum of five years of relevant professional experience in social work, case management, behavioral health, family services, or clinical services.
  • A Master's degree in Social Work or a related human services field is preferred.
  • Language proficiency to meet client needs is advantageous but not mandatory.
