Salary: The estimated salary for this role is $34.81-$62.54 per hour, based on experience and qualifications.

Job Description: As a Pediatric RN Care Manager, you will assess and coordinate medical services for complex populations in pediatric settings. Your responsibilities include developing personalized care plans, evaluating service needs, coordinating between members and care providers, and monitoring progress towards care plan goals.

Required Skills and Qualifications: To be successful in this role, you should have a Graduate from an Accredited School of Nursing or a Bachelor's degree, with 4-6 years of related experience. You must also possess a RN - Registered Nurse license or NP - Nurse Practitioner certification, depending on your background.

Job Responsibilities: Some key responsibilities for this role include:

  • Evaluating member data to identify trends and improve operating performance and quality care
  • Collaborating with healthcare providers to facilitate member services and determine revised care plans
  • Providing education to long-term care members and their families/caregivers on disease processes and care options
  • Acting as liaison and member advocate between members, physicians, and facilities/agencies

Work Environment: This role requires local travel in Austin, TX for member visits, approximately 3 days per week.
