Overview:

The ConcertoCare Advanced Practice Provider (Nurse Practitioner and Physician Assistant) is responsible for delivering in-home clinical services to patients, supported by a comprehensive multidisciplinary care team.

Our Advanced Practice Providers cultivate meaningful relationships with patients and their families, ensuring that care is aligned with the patients' values and needs while promoting patient-centered strategies that guide our care teams.

This role is perfect for clinicians who desire to practice at the top of their license, appreciate a collaborative approach to care, and are eager to contribute to our innovative, value-based clinical model aimed at serving patients with complex needs who may not be adequately supported by conventional healthcare systems.


This dynamic position allows for flexible clinical care delivery through a combination of in-person visits at patients' homes and virtual consultations from your own location, with the backing of nurses, community health workers, medical assistants, nurse case managers, clinical social workers, and clinical pharmacists.

Additionally, this role offers professional growth through dedicated clinical education programs and real-time support from our expert staff in geriatric medicine, palliative care, geriatric psychiatry, clinical pharmacy, care management, and social determinants of health and health equity.


Responsibilities of the Advanced Practice Provider:


1. Deliver compassionate, individualized, and holistic patient care, including comprehensive care, urgent care, and chronic condition management that aligns with a value-based care delivery model, reflecting appropriate patient risk stratification and integrating with multidisciplinary team services.


2. Collaborate with the multidisciplinary team to provide transitions of care support, address social determinants of health, conduct behavioral health evaluations and management, and manage complex medication regimens as clinically appropriate.


3. Provide exceptional clinical care both in-person and virtually, with in-person settings including private residences, congregate living facilities, ambulatory clinics, and skilled nursing facilities.


4. Integrate clinical practices, data dashboards, and operational processes that support the achievement of company performance goals into daily clinical workflows.


5. Promote and engage in patient experience initiatives and efforts.


6. Maintain comprehensive medical records, ensuring timely and accurate clinical documentation and coding for encounters, up-to-date medication reconciliation, and recording of patient care-related activities and communications.


7. Actively participate in care team huddles, operational meetings, organizational meetings, and clinical partnership meetings as required, including preparation and follow-up activities.


8. Collaborate with internal care team members and external providers to support care coordination, including goals of care.


9. Adhere to medical policies, protocols, criteria, and clinical guidelines to ensure best practices are maintained for effective and efficient care delivery.


10. Comply with all applicable laws and regulations, including adherence to compliance policies, procedures, and standards of conduct.


11. Serve as a growth and engagement ambassador by initiating, developing, and sustaining relationships with patients, caregivers, congregate living facility staff, and clinical partners.
