Nurse Practitioner

We are seeking a highly skilled and compassionate Registered Nurse to join our team at Advocate Aurora Health. As a Registered Nurse, you will play a vital role in providing high-quality patient care and contributing to the success of our healthcare team.
Key Responsibilities- Utilize the nursing process to assess patient needs, develop individualized care plans, and evaluate the effectiveness of care.
- Anticipate and recognize changes in patient condition, status, and need for care, and take appropriate actions to ensure seamless care transitions.
- Implement strategies to reduce patient risk and increase patient safety, and collaborate with the healthcare team to coordinate patient-centered care.
- Actively coordinate discharge plans, applying evidence-based practice to deliver high-quality patient care.
- Provide ongoing evaluation of patient, team, and unit outcomes, and take action to improve processes and patient outcomes.
- Demonstrate effective communication, feedback, and conflict resolution skills, and promote collaboration among healthcare team members.
- Promote a culture of safety, identifying threats to patient safety and intervening to prevent patient harm.
- Registered Nurse license issued by the state in which you practice.
- Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association (AHA).
- Completion of an accredited or approved program in Nursing.
- Typically requires 1 year of experience in clinical nursing.
