Job Summary

The Home Health RN provides hands-on care in the home for a group of patients consistently using the nursing process and clinical standards and protocols. This role involves overseeing the activities of the multi-disciplinary team that provides care to the patient, reviewing patient visits weekly, and communicating caseload and patient care issues to the Home Care Supervisor.

Responsibilities
  • Provide hands-on care in the home for a group of patients using the nursing process and clinical standards and protocols.
  • Oversee the activities of the multi-disciplinary team that provides care to the patient.
  • Review patient visits weekly and provide a plan to the Patient Care Supervisor and team assistant.
  • Inform the scheduler of any schedule changes and communicate caseload and patient care issues to the Home Care Supervisor.
Requirements

Required:

  • Associate's or Diploma in Nursing.

Preferred:

  • Bachelor's in Nursing.
  • 1 year of Home Care experience.
