About the Role

We are seeking a detail-oriented and organized individual to fill the role of Patient Care Coordinator. As a key member of our team at Sentido Health, you will be responsible for ensuring patients' needs are met by coordinating treatment plans and follow-ups.

In this dynamic environment, you will work closely with physicians, dietitians, staff, patients, insurance companies, and referral sources to deliver exceptional patient care and outcomes.

Key Responsibilities:

  1. Insurance Eligibility and Coverage: Determine insurance eligibility and coverage for various plans and patient-specific situations.
  2. Authorization Requests: Submit accurate and timely authorization requests to insurance.
  3. Supply Needs Assessment: Communicate effectively with patients and families to assess supply needs.
  4. Nutrition Risk Screening: Screen/rescreen patients for nutrition risk according to established criteria.
  5. Data Collection and Referrals: Document data collection results and make referrals to the Dietitian as needed.
  6. Collaboration with Healthcare Professionals: Collaborate with physicians and dietitians to address patient needs and document required information.
  7. Patient Outcomes: Work directly with staff, patients, insurance companies, and referral sources to ensure optimal patient outcomes.
  8. Communication and Documentation: Thoroughly document communication and action plans.
  9. Troubleshooting: Troubleshoot product sourcing issues if applicable.
  10. Special Projects: Participate in special projects as assigned.
  11. Order Management: Process customer orders, requests, and information updates into a patient order management system.
  12. Order Changes and Renewals: Perform Order Changes and Renewals.

Requirements:

  • An associate degree or equivalent qualification.
  • Able to work in a fast-paced, self-directed environment.
  • Proficient in using Excel, Word, PowerPoint, and nutrition software – Brightree experience is advantageous.

Work Environment:

This is a full-time position, working Monday through Friday, 8:00 a.m. to 5 p.m., with on-call responsibilities.
