About the Role

Copper Springs is a 72-bed behavioral hospital dedicated to providing exceptional care to adults, adolescents, and their families. As a Discharge Coordinator, you will be part of our clinical services team, driving a Clinical First attitude to fulfill our mission of Changing Peoples Lives.

Key Responsibilities
  • Provide aftercare coordination for patients requiring ongoing medical/psychosocial care following discharge
  • Create and maintain up-to-date aftercare coordination community resources
  • Display active involvement in treatment planning, setting up aftercare appointments as identified by the patient's provider and therapist
  • Provide resource information to patients and families for future care needs
  • Actively communicate with clients, families, aftercare providers, and referral sources
  • Prepare detailed Discharge Summaries and Contracts for each patient
  • Collaborate with the interdisciplinary team, including physicians, utilization review, and nursing staff members
  • Complete all hospital and client documentation thoroughly, timely, and in accordance with hospital standards
  • Demonstrate crisis intervention and de-escalation skills, acting as a member of the team during crises
  • Maintain confidentiality and adhere to HIPAA regulations
  • Stay up-to-date with clinical regulations, including TJC, Medicaid, and state and federal laws
Requirements
  • Bachelor's degree in social work, counseling, psychology, or relevant state licensure
  • At least three years of clinical experience within an inpatient behavioral health setting
  • Prior experience with psychiatric and chemical dependency patients
  • Unencumbered clinical license if eligible/needed per state regulations
  • CPR certification and Crisis Prevention Training (CPI) within 30 days
