At GuideStar Eldercare, we are seeking a highly skilled and compassionate Bi-Lingual Nurse Practitioner to join our team of dedicated healthcare professionals. As a Nurse Practitioner at GuideStar Eldercare, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.
About the RoleWe are looking for a Bi-Lingual Nurse Practitioner who is passionate about providing high-quality, patient-centered care to individuals with dementia, neurocognitive impairments, and/or psychiatric conditions. As a member of our multidisciplinary team, you will work closely with our clinical staff to develop and implement individualized care plans that promote patient safety, functionality, and dignity.
Responsibilities- Travel daily to assigned facilities from your home
- Provide initial evaluations, continuing therapy services, and cognitive testing
- Document the need and extent of services for each patient
- Create and maintain positive professional relationships with client facilities, patients, and their families
