Job Summary

The Licensed Practical Nurse (LPN) provides professional nursing care for residents of all ages in long-term care, under the supervision of a registered nurse, advanced practice provider, or physician. This role requires a strong commitment to delivering high-quality patient care and working collaboratively with a multidisciplinary team.

Responsibilities

As an LPN, you will be responsible for:

  • Providing direct patient care, including administering medications, wound care, and vital sign monitoring
  • Conducting thorough assessments of patients to identify their physical and emotional needs
  • Developing and implementing individualized care plans to meet the unique needs of each patient
  • Collaborating with the healthcare team to ensure seamless care transitions and effective communication
  • Participating in quality improvement initiatives to enhance patient outcomes and satisfaction

Requirements

To be successful in this role, you will need:

  • A current LPN license in the state of practice
  • Graduation from an accredited practical nursing program
  • At least 6 months of experience as an LPN in a long-term care setting
  • Ability to work effectively in a fast-paced environment and prioritize tasks efficiently
  • Strong communication and interpersonal skills, with the ability to work collaboratively with patients, families, and healthcare team members
