Job Summary

We are seeking a skilled and compassionate Registered Nurse to join our Home Health team at HCA Midwest Healthcare at Home. As a Home Health RN, you will provide high-quality, patient-centered care to individuals in the comfort of their own homes.

Responsibilities
  • Assess patients' physical, psychosocial, and environmental needs, and develop individualized care plans to address these needs.
  • Provide skilled nursing care, including wound care, medication management, and education to patients and their families.
  • Collaborate with the interdisciplinary team to ensure seamless care coordination and communication.
  • Supervise and provide clinical direction to home health aides and LPNs/LVNs to ensure quality and continuity of services.
  • Participate in on-call rotation and emergency call as needed.
Requirements
  • Basic Cardiac Life Support certification required within 30 days of employment start date.
  • Registered Nurse licensure and Associate Degree or Registered Nurse Diploma required.
  • 1 year of experience in home health or a related field preferred.
