The Terraces of Phoenix, a HumanGood community, is seeking a skilled Clinical Liaison to join our Skilled Nursing team. As a key member of our team, you will be responsible for generating business from referral sources to meet our revenue and census goals.
This role involves developing relationships with community hospitals, physician offices, community health clinics, and other community outreach programs. You will contribute to the success of HumanGood by coordinating external professional relationships for the community, with a focus on Skilled Nursing components of senior living, to achieve the highest possible occupancy within budgetary and regulatory guidelines.
Key Responsibilities:
- Develop and maintain relationships with referral sources to meet revenue and census goals
- Conduct outreach and marketing efforts to promote our community's services
- Collaborate with internal teams to ensure seamless care transitions and high-quality patient outcomes
- Stay up-to-date on industry trends and best practices in Skilled Nursing and senior living
Requirements:
- Two or more years of experience in a related field, preferably in a long-term care setting
- Current or prior Admissions, Clinical, or Post-Acute Liaison experience within a skilled nursing facility preferred
- Clinical license such as RN, LPN, RT, IT, or SLP preferred
- Bachelor's Degree, Medical or Business education, or training preferred
- Valid driver's license
