About the Role:

The Licensed Clinical Social Worker will assist patients and families in coping with problems resulting from severe or long-term illness, and with difficulties in recovery and rehabilitation. You will work closely with the healthcare team to develop comprehensive treatment plans and provide support to patients and their loved ones.

Responsibilities:
  • Assist patients with community resource planning for identified long-term care needs
  • Help patients obtain information regarding advance directives
  • Update case records
  • Contribute to the development of comprehensive treatment plans
  • Provide education and support to patients and their families
  • Identify gaps in community resources and stimulate resource development and/or improvement
Requirements:

To be successful in this role, you will need:

  • A Master's Degree from a school of Social Work accredited by the Council of Social Work Education
  • One year of social work experience in a healthcare setting
  • Current CPR certification
  • Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation
  • License as a LCSW from the State Board of Social Work (CO)
