We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our team at Sky View Rehabilitation and Health Care Center. As a skilled nursing facility, we provide high-quality care to our residents, and we are looking for an individual who shares our commitment to excellence.

Job Summary

The LPN will be responsible for providing direct resident care, including administering medications, monitoring vital signs, and documenting resident information. The ideal candidate will have excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced environment.

Responsibilities
  • Provide daily support to the unit leadership and respond appropriately to resident and family needs within the LPN scope of practice.
  • Use the nursing process to set priorities and deliver quality direct resident care, making decisions that promote the safety and quality of care.
  • Perform and administer standard nursing treatments and medications, observing residents for desired or unwanted effects.
  • Communicate and collaborate with the care team to revise and update the Nursing Care Plan or any pertinent findings, including lab and test results, incidents, and concerns.
  • Monitor and document vitals, treatments administered, and residents' responses.
  • Prepare, organize, oversee, monitor, and evaluate the delivery of care for resident CNA assignments and activities, considering the category of needs and competency of personnel.
Requirements
  • Graduate of an approved school of nursing.
  • Licensed and currently registered in good standing in New York as an LPN.
  • Dedicated to providing extraordinary, clinically sound, and compassionate care.
  • Must possess a spirit of cooperation, caring, and enthusiasm to create an atmosphere conducive to rehabilitation and optimal resident health.
  • Genuinely interested in helping ill, injured, disabled, elderly, emotionally upset, and sometimes challenging residents.
  • Have excellent communication, customer service, and organization skills.
  • Work well under pressure and willing to work harmoniously with all staff.
  • Covid-19 vaccinations are encouraged but not mandated; we continue to offer COVID-19 vaccination based on eligibility guidance from CDC.
