Job Title: LPN - Hospice Care Specialist

Agrace HospiceCare is seeking a compassionate and skilled LPN to join our interdisciplinary team as a Hospice Care Specialist. In this role, you will provide patient-centered care to individuals and their families, working collaboratively with our team to ensure the highest quality of life.

Responsibilities:
  • Provide nursing care to patients and families, including administering medications, wound care, and other treatments as needed.
  • Conduct patient assessments, develop and implement care plans, and document patient progress.
  • Collaborate with the interdisciplinary team to ensure comprehensive care and address patient needs.
  • Communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination.
Requirements:
  • Graduation from an accredited LPN program.
  • Current, unrestricted LPN licensure in the state of employment.
  • One year of experience as an LPN, preferably in hospice or palliative care.
  • Valid driver's license, auto insurance, and access to a reliable vehicle.
