Ambulatory Clinic RN

The Staff RN-Ambulatory Clinics plays a vital role in providing high-quality patient care in a dynamic and collaborative environment. As a key member of the interprofessional team, this role involves collecting and assessing patient data to develop and execute individualized care plans. The Staff RN-Ambulatory Clinics utilizes evidence-based practice to drive clinical decisions and improve patient outcomes. This role requires strong communication and coordination skills to ensure seamless care transitions across settings. Key responsibilities include patient care coordination, direct patient care, patient education, and telephone triage/eVisits. The ideal candidate will possess a strong clinical background and a passion for delivering exceptional patient care.
Responsibilities- Collect and assess patient data to develop and execute individualized care plans
- Utilize evidence-based practice to drive clinical decisions and improve patient outcomes
- Provide patient education and support to promote health and wellness
- Participate in continuous quality improvement initiatives to enhance patient care
- Collaborate with the interprofessional team to ensure seamless care transitions
- Current state of Wisconsin Registered Nurse License or Multi-state Nursing License from a participating state in the NLC (Nurse Licensure Compact)
- American Heart Association (AHA) Basic Life Support for the Healthcare Provider (BLS) or an AHA approved equivalent
- At least 1 year of relevant nursing experience
