Job Summary
HarmonyCares is a leading provider of home-based primary care services, and we are seeking a skilled Hospice Registered Nurse to join our team. As a Hospice Nurse Specialist, you will provide high-quality, compassionate care to patients in their homes, working closely with our interdisciplinary team to develop and implement individualized care plans.
Responsibilities
- Assess and evaluate patient needs, identifying mutually agreed-upon goals for care plans
- Report patient status, updates, and discharge planning to clinical leadership and attending physicians
- Perform annual on-site visits to observe and assess aide performance
- Attend staff meetings and educational in-services as needed
- Ensure the use of the 4Ms (What Matters to the patient, Medications, Mentation, and Mobility) and provide Age-Friendly Care
Qualifications
- A current, unencumbered State Professional Nurse License
- Valid driver's license and good driving record
- Ability to work in a field setting, making sound nursing judgments
- Experience with Microsoft Office
This is a per-visit position, with compensation ranging from $65 to $250 per visit, depending on the type of visit completed.
