Responsibilities

  • Assess and evaluate patient needs, identifying mutually agreed-upon goals for care plans
  • Report patient status, updates, and discharge planning to clinical leadership and attending physicians
  • Perform annual on-site visits to observe and assess aide performance
  • Attend staff meetings and educational in-services as needed
  • Ensure the use of the 4Ms (What Matters to the patient, Medications, Mentation, and Mobility) and provide Age-Friendly Care

Qualifications

  • A current, unencumbered State Professional Nurse License
  • Valid driver's license and good driving record
  • Ability to work in a field setting, making sound nursing judgments
  • Experience with Microsoft Office

This is a per-visit position, with compensation ranging from $65 to $250 per visit, depending on the type of visit completed.
