Hospice Nurse RN

At Three Oaks Hospice, we are committed to providing exceptional care and compassion to our patients and their families. We are seeking a dedicated and compassionate Registered Nurse RN to join our team of hospice care professionals.
Job Summary:The primary function of this role is to administer skilled nursing care to clients in their place of residence, coordinate care with the interdisciplinary team, patient/family, and referring agency, and assume responsibility for coordination of care.

- Coordinate care with the interdisciplinary team, patient/family, and referring agency
- Assume responsibility for coordination of care
- Provide effective support to patients and families
- Collaborate with the interdisciplinary team to develop and implement individualized care plans
- Graduate of an accredited Associate or Bachelor's degree in Nursing
- Current state license as an RN and current state driver's license
- Two years of experience as an RN in a clinical care setting; hospice or home health experience preferred
- Current CPR certification
- Proficient skill using a computer; Microsoft Office suite experience preferred
- Strong interpersonal skills and ability to provide effective support to patients and families
