Job Summary:

As a Home Health Assistant, you will play a vital role in coordinating and implementing paraprofessional services for Kaiser members. Your expertise will be essential in recommending referrals to appropriate resources and agencies within the Kaiser system and in the community.

Key Responsibilities:
  • Provide direct nursing service and care to patients in their homes, as delegated by the home health Registered Nurse (RN) and/or physical/speech/occupational therapist (PT/ST/OT), and in compliance with the plan of care.
  • Assist patients with prescribed home exercise programs and be directly responsible to the case manager.
  • Monitor, educate, and collaborate with patients/caregivers and case managers in the home setting.
  • Network and coordinate with the Home Health multidisciplinary team to best meet the needs of the patient.
  • Provide simple household chore services essential to healthcare at home, as assigned.
  • Understand and apply universal infection control procedures, maintaining a clean, safe, and healthy environment for patients assigned to your care.
  • Prepare equipment and materials for treatments, observing aseptic technique as required.
  • Observe and document patient status and care/service furnished.
  • Recognize and handle emergency situations as directed until help arrives.
  • Educate patients and caregivers in appropriate self-care techniques.
  • Participate in multidisciplinary team conferences and attend continuing education and in-service training programs to keep current on skills and patient care techniques/needs.
Requirements:
  • Minimum one (1) year community health and geriatric nursing experience.
  • Employed in community health service to geriatric population within the last twenty-four (24) consecutive months.
  • Bachelor's degree in a healthcare field with home care aide/nurse aide certification, or graduate from an approved course in practical nursing.
  • Licensed Practical Nurse (Hawaii).
  • Basic Life Support from the American Heart Association.
