Key Attributes:

Required Licenses/Certifications:
Registered Dietitian/Nutritionist (RD/RDN)
Core Responsibilities:
- Educate elderly patients on the use of our mobile applications and medical devices, providing troubleshooting assistance as needed.
- Collaboratively design personalized care plans and set behavioral goals with each patient, ensuring HIPAA compliance in documentation.
- Implement established Standards of Practice and Professional Performance in diabetes care to guide educational interventions.
- Engage with primary care providers and other healthcare professionals to ensure comprehensive patient care.
- Utilize iHealth Unified Care software to monitor patient outcomes and facilitate routine goal setting and educational support.
- Assist patients through our online platform during scheduled shifts and visits.
- Collaborate with cross-functional teams to enhance digital health solutions for chronic disease management.
- Potential to mentor colleagues, contribute to clinical protocol enhancements, or participate in internal research initiatives.
- Depending on expertise and interest, create educational materials such as blog posts, videos, or handouts that align with AADE and AHA standards.
- Additional responsibilities as assigned.
