RN Case Manager

We are seeking a highly skilled and compassionate RN Case Manager to join our team at the University of New Mexico's Department of Pediatrics, Continuum of Care Division. As a RN Case Manager, you will play a critical role in providing high-quality patient care and ensuring seamless transitions between healthcare settings.
Job Summary:The RN Case Manager will be responsible for assessing, planning, implementing, and evaluating patient care in a variety of settings. This includes coordinating with healthcare providers, families, and patients to ensure comprehensive care and addressing complex issues related to developmental disabilities. The ideal candidate will possess excellent communication and problem-solving skills, with the ability to work independently and as part of a multidisciplinary team.
Key Responsibilities:- Assess and plan patient care in various settings, including clinics and community programs
- Coordinate with healthcare providers, families, and patients to ensure comprehensive care
- Address complex issues related to developmental disabilities, including education and training
- Manage and coordinate insurance issues related to Neurology, CP, and CP Ortho Clinics
- Complete mortality reviews and design/present trainings on various developmental disability issues
- Associate's degree in Nursing; at least 1 year of experience directly related to the duties and responsibilities specified
- State of New Mexico Licensed Registered Nurse or licensure pending as documented by temporary licensure OR current RN licensure from a state participating in the multistate privilege to practice compact with New Mexico
- Knowledge of DD, New Mexico DD Waiver and I/DD systems, anticonvulsant medications, Vagal Nerve Stimulator (VNS), CP, CP Ortho, Neurology, and Ketogenic Diet
- Experience in Case Management and designing/presenting trainings on I/DD topics
