At Professional Case Management, we are seeking a highly skilled and experienced Senior Case Manager to join our team. As a Senior Case Manager, you will be responsible for providing direct care and case management to assigned clients, while also supervising and mentoring a team of RN Case Managers.
Key Responsibilities:
- Provide direct care and case management to assigned clients, ensuring adherence to Agency policies and regulatory requirements.
- Supervise and mentor a team of RN Case Managers, providing guidance and support to ensure high-quality care is delivered.
- Participate in all Agency performance improvement initiatives, including quarterly medical record review and data collection.
- Collaborate with staffing coordinators to ensure appropriate staffing and scheduling of personnel.
- Ensure adherence to nursing delegation guidelines and Agency policies.
Requirements:
- Graduate of a state-approved school of professional registered nursing.
- Minimum of two years of nursing experience, including one year in home care or a closely related field.
- One year of supervisory and/or case management experience preferred.
