Job Title: RN Case Manager

Verovian Healthcare Recruitment Agency is seeking a dedicated and organized RN Case Manager to join a dynamic team in Moses Lake, Washington, and support the coordination of care across various healthcare settings.

Job Summary

The RN Case Manager coordinates and manages patient care from admission through discharge. This role involves collaborating with physicians, nurses, social workers, and other healthcare professionals to develop and implement individualized care plans that promote the best possible outcomes for patients.

Key Responsibilities
  • Coordinate and manage patient care throughout the continuum of services, ensuring timely and appropriate care.
  • Develop individualized care plans in collaboration with patients, families, physicians, and other healthcare providers.
  • Serve as a liaison between the patient, family, and healthcare team to ensure clear communication and understanding of care plans.
  • Monitor and evaluate patient progress, adjusting care plans as necessary to achieve optimal outcomes.
  • Conduct assessments to identify patient needs, resources, and barriers to care.
  • Facilitate discharge planning, ensuring patients have the necessary follow-up care, resources, and support upon leaving the hospital.
  • Advocate for patient needs, ensuring they receive the appropriate level of care and services.
  • Collaborate with social workers, therapists, and other healthcare professionals to provide comprehensive care.
  • Ensure compliance with regulatory requirements, hospital policies, and best practices.
  • Participate in quality improvement initiatives and case reviews to enhance patient care delivery.
Candidate Requirements
  • Current Registered Nurse (RN) license in Washington.
  • Bachelor of Science in Nursing (BSN) is preferred; Associate Degree in Nursing (ADN) is considered with relevant experience.
  • Minimum of 2 years of clinical nursing experience, preferably in case management, discharge planning, or a related field.
  • Strong understanding of care coordination, discharge planning, and case management processes.
  • Excellent communication, organizational, and problem-solving skills.
  • Ability to work independently and as part of a multidisciplinary team.
  • Certification in Case Management (CCM) preferred.
