The RN Case Manager coordinates and manages patient care from admission through discharge. - Coordinate and manage patient care throughout the continuum of services, ensuring timely and appropriate care.
- Develop individualized care plans in collaboration with patients, families, physicians, and other healthcare providers.
- Serve as a liaison between the patient, family, and healthcare team to ensure clear communication and understanding of care plans.
- Monitor and evaluate patient progress, adjusting care plans as necessary to achieve optimal outcomes.
- Conduct assessments to identify patient needs, resources, and barriers to care.
- Facilitate discharge planning, ensuring patients have the necessary follow-up care, resources, and support upon leaving the hospital.
- Advocate for patient needs, ensuring they receive the appropriate level of care and services.
- Collaborate with social workers, therapists, and other healthcare professionals to provide comprehensive care.
- Ensure compliance with regulatory requirements, hospital policies, and best practices.
- Participate in quality improvement initiatives and case reviews to enhance patient care delivery.
